Direct Care Memberships
It is like a Gym Membership for Healthcare!
WHAT IS DPC?
Direct Care is an affordable membership approach to your medical or psychiatric mental health care.
Instead of paying separately for each covered visit, you pay a predictable monthly membership fee. Like a gym membership, your payment keeps your membership active and gives you access to the services included in your selected program.
Insight Health Clinic offers two separate membership programs:
Direct Primary Care for medical services.
Direct Psychiatric Care for psychiatric mental health services.
Both programs offer an unlimited-visit option and a six-visits-per-membership-year option.
WHY CHOOSE DIRECT CARE?
Having health insurance does not always mean routine care is affordable. With a high-deductible plan, you may pay substantial amounts out of pocket before insurance begins covering certain services. Co-payments and co-insurance can add to the expense of regular appointments.
DPC offers another way to pay for covered care at our practice. Your monthly fee makes those costs easier to understand and budget. For covered membership services, there are no insurance claim forms, insurance co-payments, co-insurance, or deductibles.
Membership can be helpful whether you have no health insurance or have insurance with a high deductible.
HOW IS OUR MEMBERSHIP APPROACH DIFFERENT?
Direct Care emphasizes a consistent relationship with your healthcare provider, individualized attention, and time to address your concerns.
Our programs focus on wellness, prevention, and management of acute or chronic medical and psychiatric mental health conditions. We draw on Integrative, Functional, and Conventional nursing and medical approaches when appropriate.
Regular follow-up gives us opportunities to review medications, discuss symptoms, monitor progress, and adjust your care plan.
Members may request same-day or next-business-day appointments, subject to appointment availability and clinical needs. Membership does not provide continuous, emergency, or after-hours access.
TWO AFFORDABLE MEMBERSHIP OPTIONS
Choose the option that fits your anticipated care needs:
OPTION A — UNLIMITED VISITS
Includes unlimited covered visits within your selected Medical or Psychiatric Mental Health membership program.
OPTION B — SIX VISITS PER MEMBERSHIP YEAR
Includes six covered visits within your selected program during the membership year.
Both options are monthly memberships. The monthly fee remains due even during months when you do not have a visit. Additional visits under Option B and services outside the membership are subject to the membership agreement.
IN-OFFICE AND TELEHEALTH CARE
Members may receive covered services in our Tempe office or through secure Telehealth Video Visits, with no additional telehealth charge.
Video visits offer a convenient way to discuss medical or mental health concerns, manage medications, or follow up after in-office care. Some concerns require an in-office examination, testing, or a procedure.
Telehealth visits count toward the six-visit allowance when you select Option B.
WHAT SERVICES DOES MEMBERSHIP OFFER?
Depending on your selected program and membership agreement, services may include:
Annual wellness exams.
Management of acute or chronic conditions.
Medication management and refills.
Psychiatric mental health treatment and psychotherapy.
Selected minor procedures and in-office testing.
Annual screening laboratory testing for eligible adults after six months of membership.
Our broader wellness offerings may also include coaching, weight loss and management services, and access to discounts on medications, IV services, and supplements.
Not every service is included in every membership. Your agreement identifies covered services, eligibility requirements, discounted services, exclusions, and additional charges. Psychiatric medication-management or psychotherapy visits exceeding 45 minutes incur an additional $25 for each additional 15 minutes.

NO HEALTH INSURANCE? NO PROBLEM!
Traditional health insurance is not required to receive covered services through an eligible Direct Care membership.
Our memberships provide an affordable way to maintain routine medical or psychiatric mental health care through a predictable monthly payment.
For patients with high-deductible insurance, membership can complement their coverage: DPC pays for covered care at our practice, while insurance may help with hospitalization, specialists, imaging, and other services outside membership, according to the insurance plan’s benefits.
IS DPC HEALTH INSURANCE?
No. Membership pays for specified services at our practice. It does not replace comprehensive health insurance.
Emergency care, hospitalization, outside specialists, imaging, and other excluded services remain separate expenses. We encourage members to maintain insurance for these needs.
The current membership agreement excludes people enrolled in Medicare or Medicaid. This restriction applies to the membership program, separate from our insurance-based care.
HOW DO I GET STARTED?
